Randomly my reference layers break. To me it's no rhyme or reason why. Maybe this video can shed light on the issue as to why....

I've seen issues with Reference layers but not where it would offset items like that. I suspect the issue is an errant keyframe in one or more smart bone actions for the referenced layer.
Otherwise, it may be an offset keyframe in the Reference layer on the Mainline that's throwing it out of sync. To fix this, you can select the referenced layer and r-click on the 'bad' channel's name in the Timeline and choosing Sync, or you can resync the entire layer by r-clicking it in the Layers window and choosing Sync All Channels to Original. This should match the item to the orignal.
If that's not it, is it possible to upload a stripped-down version of the file for someone to look at?
FYI, the big problem with Reference layers is when ALL of the channels can get randomly keyframes, whether they need it or not. To correct the problem, I need to resync all the 'bad' channels individually, which is a real hassle after I've already started animating the layer. In this case, I typically wind up converting the Reference to a regular layer (r-click and select Break Layer Reference) so it works more reliably. Anyway, this one is a known and previously reported issue, so hopefully, a fix is in the works.

Additional info: Normally, you can tell when a Reference layer is out of sync because the layer's icon turns red. When it's synced, the icon goes back to its regular color.
I can't tell if this has happened to any of your artwork layers because the group layers are collapsed. Troubleshooting this setup should be easier if you can open the group layers to show all the child layers.
