Re-rendered after unchecking Enable Multi-Threaded rendering and it worked.

So - I guess it's a bug.

There is no .mp4 in the render cache. Also I have no problem previewing files that I have actually rendered.

Hmmm. When I select the Preview Animation option from the menu nothing happens. Just nothing.
